General:
- Changed OpenBSD to use -pthread rather than -lpthread on compiles
  and links as suggested by Adam Kropelin.
- Made the above change to NetBSD -- hoping it will work.
- Merged the latest version of the manual (it has a bit of 3.9.x
  stuff).
