	Changed the order in which the power level is calculated to improve 
	accuracy and avoid overflows.
	Also in order to be more consistent the smoothing buffer now gets the
	square root of the power.

	* src/smooth.c: Undone the change from 2000-09-09 such that the gain is
	applied from the start and there is no wait for the buffer to fill, there
	are songs where the start is not quiet and this created a sudden jump
	after the song was already playing.
	
	* src/cfg.c: Changed the default maximum gain to 10.0 instead of 15.0
